Straight neck and forward head posture describe posture and spine alignment rather than formal diagnoses
When the ears sit forward of the shoulders from a side view, or when an X-ray shows reduced natural curvature of the cervical spine, it is commonly called forward head posture or straight neck (text neck). Spending long hours looking at screens or working with the head bowed can easily cause stiffness in the muscles of the back of the neck and shoulders.
However, many people have no pain even with the same neck curve or posture. Rather than determining the cause of pain from a single image alone, it is necessary to examine which postures and movements trigger discomfort.
Discomforts commonly experienced together
- Stiffness in the back of the neck and pulling when bending the head backward
- Heaviness around the shoulders and shoulder blades when sitting and working for long hours
- Tight, squeezing pain in the back of the head or temples by the afternoon
- Not feeling refreshed in the neck upon waking in the morning
- Waking from sleep due to neck or arm pain
- Frequently hearing from others that the back and shoulders are hunched
These items are used to organize what to discuss during a consultation and do not determine a disease based on the count.
If arm tingling and weakness are present, cervical nerve symptoms are also checked
If neck pain extends past the shoulder into the arm or fingers with tingling, or if tingling and pain frequently wake you at night, it is not viewed as a simple posture problem alone. Sensation, muscle strength, and reflexes should be checked considering the possibility of a cervical disc disorder or nerve compression.
Seek immediate medical evaluation if strength in the arms or legs suddenly drops, gait becomes unsteady, hand coordination noticeably slows such as frequently dropping objects or struggling with buttons, changes in bowel or bladder control occur, or neck pain develops after major trauma. If severe neck stiffness is accompanied by fever, or if neck and arm pain appear with chest pain and cold sweats, call 119 (Korea's emergency number) or go to an emergency room immediately.
What is checked at a Korean medicine clinic
A Korean medicine doctor asks when the pain started, the time of day it worsens, working posture and screen position, and sleeping posture. The range of bending the head forward, backward, and turning, movement of the shoulders and upper back, and tension and tenderness in the muscles around the neck and shoulders are examined.
If arm tingling or headaches are also present, the location of tingling, muscle strength, and headache patterns are also checked. If you have previous imaging studies, bring them, and they will be interpreted together with current symptoms and physical examination findings.
Treatment and lifestyle management
If clinical examination reveals no red flags and movement restriction and muscle tension in the neck, back, and shoulders appear related to pain, Chuna manual therapy, acupuncture, and pharmacopuncture may be considered according to the patient's condition. Rather than viewing the neck in isolation, treatment plans are established by reviewing upper back and shoulder movements together.
Along with treatment, raising screens closer to eye level, changing posture every 30 to 60 minutes, and consistently performing comfortable neck and back movement exercises are helpful. It is advisable to reduce the habit of repeatedly cracking the neck to make popping sounds. Treatment response is evaluated by whether working duration, sleep, and headache frequency actually change alongside pain intensity.
Frequently Asked Questions
If a straight neck is corrected, does the curve return?
There is insufficient evidence that the curve of cervical vertebrae changes distinctly through treatment or exercise. Because curve shape and pain are often separate, treatment is evaluated based on changes in current pain, movement, and daily function.
Does forward head posture lead to a cervical disc herniation?
The relationship between posture and disc disorders varies by individual, and having forward head posture does not necessarily progress to a cervical disc disorder. If arm tingling and muscle weakness develop, neurological symptoms should be evaluated.
Will changing only my pillow make it better?
A pillow suited to your sleeping posture can be helpful, but daytime working posture and activity levels must be adjusted together.
