If You Were Told Your Pelvis Is Misaligned: How Pelvic and Body Imbalances Are Evaluated

Rather than concluding pelvic and body asymmetry from external appearance alone, this guide explains criteria for evaluating lower back, pelvic, and hip joint movement alongside pain.

Visible asymmetry and pain are evaluated separately

It is common to have slightly different shoulder or pelvic heights, a skirt that twists to one side, or one shoe wearing down faster. Such asymmetries can arise from daily habits or differences in muscle use, but they do not necessarily indicate pain or disease in themselves.

Therefore, a Korean medicine doctor does not conclude upfront that "you hurt because your pelvis is misaligned." We examine together in which postures and movements the lower back, pelvis, and buttocks actually hurt, and how functional activities such as walking, sitting down, and standing up are performing.

Changes commonly noticed

  • Shoulder heights appear uneven
  • Pants or skirts twist to one side
  • One edge of a shoe wears down noticeably more
  • Standing with weight on one leg feels more comfortable
  • Discomfort around the lower back, pelvis, or tailbone when sitting or standing for long periods
  • Pain in one buttock or groin when walking

These items are used to organize what to discuss during a consultation. The condition is not determined based on the number of matching items.

When other underlying causes must be evaluated first

If you limp or struggle to put on socks due to buttock or groin pain, issues within the hip joint itself must be evaluated. If a growing child shows shoulder and pelvic height differences along with one side of the back bulging upward when bending forward at the waist, an evaluation for scoliosis is needed.

Seek prompt medical evaluation if you have difficulty bearing weight due to pelvic or buttock pain after a fall; if legs lose strength, changes in bowel or bladder control occur, or numbness develops in the buttocks or perineum; if severe pain is accompanied by fever; if one leg is swollen and painful; or if a growing child suddenly begins to limp. If lower abdominal pain or unusual vaginal bleeding is also present, gynecological evaluation is also necessary.

What is checked at a Korean medicine clinic

A Korean medicine doctor checks the painful area, when it began, working postures, exercise habits, and history of childbirth or trauma. Standing posture, gait, bending forward and backward at the waist, hip joint movement, single-leg standing, and strength in the buttock and trunk muscles are examined.

If a difference in leg length is suspected, we attempt to distinguish whether it is a true anatomical bone length difference or a functional difference arising from muscle tension or posture. Because physical examination alone has limitations, imaging studies are guided if needed. Imaging is considered when red flags are present or when the results would change the direction of treatment.

Treatment and lifestyle management

If clinical examination reveals that restricted movement and muscle tension in the lower back, pelvis, and hip joints appear related to pain, Chuna manual therapy, acupuncture, and pharmacopuncture may be considered according to the patient's condition. Rather than aiming to make external appearance perfectly symmetrical, Chuna manual therapy evaluates treatment response based on changes in pain, movement, and daily function.

It is beneficial to reduce habits of crossing only one leg or shifting weight onto only one leg, and to combine exercises that strengthen buttock and trunk muscles. Symptoms such as menstrual pain, indigestion, and weight changes are evaluated separately for their own causes, apart from pelvic alignment.

Frequently Asked Questions

Does a misaligned pelvis cause many other diseases?

The relationship between visible pelvic asymmetry and pain or disease differs from person to person, and there is not enough evidence to say that asymmetry alone causes other diseases. Actual symptoms and function need to be checked together.

Will Chuna manual therapy make my pelvis symmetrical?

The goal of Chuna manual therapy is not to make the body look perfectly symmetrical but to improve pain, movement, and daily function. Treatment response is checked by the same criteria.

I was told my legs are different lengths.

It is necessary to distinguish a true difference in bone length from one caused by posture or muscle tension, and the exact length is confirmed with imaging. If the difference is small and there are no symptoms, separate treatment may not be needed.
